Summary: This capacity building effort will work with sickle cell patients to: 1) increase knowledge and awareness of useful interventions on chronic disease management; 2) increase their recognition of their role in the translation of research to practice; 3) train partners to become a patient activist in the process of research dissemination; and 4) engage stakeholders as partners in the design and development of a patient-centered, scalable dissemination model
